​Please arrive 10-15 minutes early to your first appointment so that your massage therapist has time to review your forms.
Or simply arrive 5 minutes early if you have printed and completed the paperwork in advance.

The door may be locked when you arrive since we are by appointment only and secure our space while in session.

Massage Therapy Health History & Release Form

Please, thoroughly complete the your health history form.  This information is very important for us to develop an effective treatment plan and to ensure that we do not use any techniques that are contraindicated or may put you at risk.  We keep all of your information secure and confidential.
